Botox Injection Technique: the PTIFA Weal Technique

Intramuscular injection of botulinum toxin A (BoNT-A) is a common treatment both aesthetically for the relaxation of wrinkles, and therapeutically for Temperomandibular Disorder (TMD) and bruxism. Studies have shown however, that when injecting the Frontalis muscle, there is no significant difference in anti-wrinkle efficacy between intramuscular (IM) and intradermal injections (ID) (Carruthers & Carruthers, 2013). Intradermal injections of BoNT-A are associated with a lower risk of adverse side effects. The use of an intradermal BoNT-A injection technique however, has only been studied within the Frontalis region.

In this blog post, Drs. Roberts reviews how to reconstitute Botox and Dysport, as well as some frequently asked questions on reconstituting Botox.

What product is used to reconstitute Botox and Dysport?

To achieve predictable results, avoid negative side effects and provide a high-level of patient care, 0.9% Preserved Saline is used which contains Sodium Chloride 9.0 mg/ml and Benzyl Alcohol 9.0 mg/ml.

In a study conducted in 2002, 100% of patients reported less pain, less discomfort when preserved saline was used, rather than with non-preserved saline (Alam, Dover & Arndt Ach. Dermatol. 138:510:2002). Benzyl alcohol acts as a mild anesthetic and kills bacteria making the reconstituted product last longer.

Botulinum Toxin (aka Botox/Dysport) has become a recognized modality for treating myofacial pain including migraines, headaches, TMD, bruxing, clenching grinding, etc. However, many dentists and physicians are unaware of the importance of incorporating EMG (electromyograph) analytics into their diagnoses and treatment planning.
